Also Dr Yewande Olupitan premiered a fantastic montage of Nigerian Diaspora and outlined the special relationship Nigeria has with Ireland in the past number of years. Association of Nigerian Doctors in Ireland Inaugural Dinner

In a bid to raise &euro;10,000 for an ambulance for the Order of Malta Ronan Corrigan, Mark Compton and Davy Glen attended a training camp in the Algarve in preparation for the Chicago Marathon. The camp ran from 12 April to 16 April 2017 and was led by Olympic and Commonwealth medal winners, Liz McColgan and John Nuttall.
The team took part in a range of activities and training sessions.
Mark Compton reveals, &ldquo;We had no idea what was around the corner. Although we expected a tough week, aqua sprints, track sessions and single leg lunges were only &lsquo;running talk&rsquo; to us. Ronan, the more serious runner of the three took to it like a duck to water and ran like an Olympian from day one. Davy and I however struggled slightly at the beginning, but after gritting our teeth and plodding away, step after step, we began to make great inroads. So much so, that by day four, the one and only Liz McColgan (British Olympian and our coach) referred to us as &lsquo;athletes&rsquo;. This was our proudest running moment to date and was probably worth the hip, calf, ankle, shin, and hamstring pain we endured for five nights and six days. We learned so much over the week and are all very excited about our running going forward.&rdquo;

Algarve Training Camp Update

The Global Medics team in Ireland is aiming to raise &euro;10,000 for an ambulance for the &lsquo;Order of Malta&rsquo; a voluntary organisation which Compliance Officer Megan Johnston volunteers for.
The Order of Malta is a major provider of first aid training, ambulance transport and community care services and assistance to other charities throughout Ireland. The ambulance currently serving the Blackrock Unit has been in the healthcare service for 17 years and if the team achieve their fundraising goal, it will be the biggest fundraising amount ever gathered by the unit &ndash; more money than the unit takes in through an average year of covering events and fundraising.
Mark Compton, Ronan Corrigan and Davy Glen have already completed a 10k run and will be taking part in the Chicago Marathon in October 2017 to raise funds for this worthy cause. Click here to donate and support the team.

Order of Malta, Blackrock Unit Fundraising

In an effort to challenge the widespread perception that locum doctors present a greater risk to patients than their permanent counterparts, new research has been conducted by the Journal of the Royal Society of Medicine. In light of these findings, we caught up with our Head of Clinical Services, Anne Anderson, to discover her thoughts on this news.
What are your views on the perception that locum doctors present a greater risk to patients than permanent doctors? 
&ldquo;The perception that locum doctors are not supported and clinically governed at the same level as substantive doctors still exists, unfortunately. It stems from the days before frameworks came along, when agencies dispatched doctors who may not have had any compliance checks, and prior to the rigorous checks now required for all doctors under revalidation.&rdquo;
The research suggests the factors that affect the quality and safety of locum practice is linked with how they are deployed and supported by the organisations who use locum doctors. Does this reflect your experience? 
&ldquo;Yes, I agree with this. While we only place doctors who pass rigorous compliance processes, it is equally important that they are supported in their working environment. A great deal of emphasis is also placed on sharing information for development and reflective purposes and it is therefore important that NHS organisations support this through our structured feedback process; if this doesn&rsquo;t happen then action can&rsquo;t be taken, and lessons can&rsquo;t be learned. This best practice is highlighted by Sir Keith Pearson in his Review of Revalidation for Doctors and is linked to revalidation and its application to locum doctors.
&ldquo;But actually, it&rsquo;s not up to hospitals to support locum doctors with clinical audits and revalidation matters; it&rsquo;s up to agencies like us to take responsibility for this and support locums when needed. We have built robust processes that help investigate allegations against our locum doctors and therefore, support our clients in their accountability.&rdquo;
What support can our locum doctors expect from us with a clinical governance issue? 
&ldquo;We have a robust clinical governance system delivered by a dedicated team of clinical professionals. Events include complaints, serious incidents, professional and competency concerns. Each event is dealt with confidentiality and with the utmost sensitivity. We mirror NHS event management processes so that we can work in partnership with our clients ensuring that we collaboratively achieve fair, appropriate and continuous improvement outcomes.
&ldquo;As the Head of Clinical Services I have clinical oversight and support from our Responsible Officer Dr June Picton to ensure that we take the most appropriate approach. We may recommend further training or reflection and ensure that the event forms part of the doctor&rsquo;s appraisal for revalidation. We also support doctors with GMC investigations, coroner&rsquo;s courts, legal matters, patient complaints and general well-being for what can be a stressful job.&rdquo;

The UK Government has updated its guidance on coronavirus testing which has become available for essential workers in England and Scotland who are self-isolating in a move that could see healthcare professionals return to work.
The National Testing Programme provides testing for the virus, which is now available to essential workers with symptoms, as well as those living in the same household who are displaying symptoms of COVID-19.
The move will help support the return to work of urgently required frontline staffand allow employers the opportunity to maximise their workforce capacity during this unprecedented time.
At this time, the testing is only available in England and Scotland.
Am I eligible for a test? 
If you or a member of your household is self-isolating because you have COVID-19 symptoms, you will be eligible for testing.
How can I get tested? 
You can select a regional test site drive-through appointment or home test kit. Home test kit availability will initially be limited but more will become available. There is good availability of regional test sites, so please select this option if you can.

You will be offered a testing appointment in your area and will need to ensure you have the following available on arrival:

COVID-19 Testing Guidance for Essential Workers

In March 2020, appraisal, revalidation and associated activities were suspended for the majority of doctors in the UK due to the COVID-19 global pandemic. This included the compiling of portfolios, recording of CPD, quality improvement and formal feedback.
In a time of national emergency, this action was taken to allow doctors time to focus their efforts on combatting the worst health crisis in living memory.
<h2>Restarting Appraisals</h2>
Due to restart in October 2020, the process of appraisal and revalidation will be slightly different to how it was before its suspension in March 2020, but will remain consistent with existing GMC guidance.
Medical Appraisal 2020 aims to;
<ul>
<li>Rebalance the appraisal processes</li>
<li>Reduce the documentary requirements for the pre-appraisal documentation</li>
</ul>
The new processes not only place emphasis on the importance of the existing purposes of appraisals by supporting your personal and professional development as a doctor, it also focuses on how your health and wellbeing have been maintained during the COVID-19 crisis, and what further support you may require.
Medical Appraisal 2020 has been designed in a way that will be appropriate for doctors in all sectors and locations. In testing, the new processes have shown that the reduction in pre-appraisal documentary requirements mean that it is possible to prepare a focused written reflection in under 40 minutes.
In preparation, appraisers will be required to update their training to ensure that they have the resources and contacts necessary to provide the support you need as a doctor. Due to the nature of the COVID-19 crisis, the restart of appraisals will be flexible for designated bodies and Responsible Officers, allowing for an appropriate response to the ongoing pandemic.
<h2>Why were doctors' appraisals suspended?</h2>
During the initial COVID-19 response, Professor Stephen Powis, National Medical Director of NHS England, informed all doctors that it would be reasonable to suspend the work that goes into the demonstration of continued competence, so that more focus could be given to what they do best.
Whilst maintaining professional standards is vitally important, it was safe to assume that doctors who are normally able to demonstrate that they keep up to date with clinical requirements and maintain fitness to practice would continue to do so. This helped to reduce the burden of non-urgent, non-clinical work, enabling doctors to use their time to rest and recharge, both mentally and physically.
For some doctors, appraisals were not suspended. In fact, in certain circumstances, they have been deemed a supportive intervention and continued in some designated bodies where they were particularly valuable in the context of the pandemic.
<h2>Why are appraisals restarting halfway through the year?</h2>
While it is appropriate to reduce the burden of demonstrating continued competence during a pandemic, it is not suitable to remove a supportive intervention that reaches every doctor and helps them to improve their patient care and plan for the future. The temporary suspension created an opportunity to review and rebalance the appraisal process to ensure it benefits both doctors and patients.
By restarting appraisals with a new process and emphasis on supporting doctors, appraisers have an opportunity to offer you support, even if you wouldn't usually ask for it. It's believed that this opportunity is needed all the more acutely because of the impact, disruption and distress that has been caused by the COVID-19 pandemic.
It is important that all doctors receive protected time and a safe space to reflect with a trained peer. Restarting appraisals now will also allow Responsible Officers to make appropriate revalidation recommendations for those doctors who are now under notice, especially if they were already ready to revalidate.
Another benefit is bringing those doctors whose appraisals were suspended back in line with those whose appraisal systems never stopped.
<h2>What action has the GMC taken around revalidation recommendations?</h2>
On 17 March 2020, the GMC announced it was going to change revalidation dates for all doctors with a revalidation recommendation due before the end of September 2020. The desired effect was to reset the date for 12 months later. This decision was taken to relieve doctors, Responsible Officers and governance teams during the pandemic.
<h2>What does this mean for doctors whose revalidation recommendation was due prior to 16 March 2021 and has been changed by the GMC?</h2>
The GMC has removed the requirement for Responsible Officers to make a recommendation about your revalidation. As a doctor, you can carry on practising as before. You'll even receive an extra year in which to complete all necessary requirements for revalidation recommendation, should you require it.
If your medical appraisal is up to date and the governance information checks reveal that you are ready to revalidate, your Responsible Officer will be able to make a positive revalidation recommendation to the GMC.
If you have received notification that a decision has been made to revalidate you, you will receive a new revalidation date for five years after your last.
You may notice that there are more than five years before your next revalidation recommendation is due. This is because your date was initially moved forwards by twelve months, and five years were added to this date.
<h2>My 2020/21 appraisal has been postponed/suspended; what should I do?</h2>
If your appraisal was due before the end of September 2020 and was postponed, your appraisal administration team has been advised to put postponed 2020/21 appraisals on hold during the period of suspension. You'll be contacted once a designated body has deemed it appropriate to restart the appraisals process.
If your appraisal was due before the end of September 2020 and was suspended, the process is slightly different. Your appraisal administration team have closed down 2020/21 appraisals, meaning the appraisal year will be missed. However, due to the nature of the COVID-19 pandemic, this missing year will be accepted and recognised by the GMC.&nbsp;
<h2>I haven&rsquo;t collected any supporting evidence during the COVID-19 pandemic; is this an issue?</h2>
The impact of the COVID-19 pandemic may have affected your ability to gather supporting evidence ahead of your appraisal, but don&rsquo;t worry. Appraisers are being trained to adjust their expectations accordingly, taking into account the impact of the health crisis.
You will still be expected to provide a focused written reflection ahead of your appraisal and then discuss your reflections on what you have learned and what changes you have made as a result. You&rsquo;ll also need to include any complaints or significant events in which you have been personally named or involved with since your last appraisal.
<h2>What will happen to the appraisal process after September 2021?</h2>
Due to the unpredictability of the current health crisis, it is impossible to predict how COVID-19 will affect the appraisal process in the future. It is hoped that, by closely monitoring and evaluating the introduction of Medical Appraisal 2020, the appraisal process will evolve so that it can be as supportive and constructive as possible for all doctors.

For locum doctors, attempting to decipher the rules surrounding IR35 can be confusing. And when your primary concerns are tending to those in your care, the last thing you need to worry about is a run-in with Her Majesty&rsquo;s Revenue and Customs (HMRC) regarding your IR35 status.
On 6 April 2021, the UK government will be extending the IR35 rules to include the private sector. To make your IR35 status clear, we have created a simple, helpful guide.
What does working inside IR35 mean? 
Being &lsquo;inside IR35&rsquo; means that, for tax purposes, you are deemed to effectively work for your end client in the same way as a permanent employee and are therefore subject to PAYE. Operating inside IR35 generally means that your employer (this could be an umbrella company or recruitment agency) deducts the appropriate taxes and National Insurance contributions from your wage &lsquo;at source&rsquo;, which means before it reaches your bank account. If you are operating inside IR35, you must ensure that you are paying the appropriate taxes over the course of a financial year. This may include paying a deemed payment (e.g. higher taxes) at the end of the tax year.
If your employment situation changes throughout the course of a financial year, your IR35 status must be reassessed.
What does working outside IR35 mean? 
Being &lsquo;outside IR35&rsquo; means that you are deemed to be operating more independently than a permanent employee would. This means that, through your limited company (also known as a personal service company), you are responsible for managing your own taxes. In addition, you will be able to pay your own salary and draw the remainder of your income as dividends.
Am I inside IR35 or outside IR35? 
Determining whether you are inside or outside IR35 comes down to how the contract between your limited company or umbrella company and the end client is perceived. It is not determined by your payment structure.
However, determining whether you are inside or outside IR35 is not down to you.
Who decides if I&rsquo;m inside or outside IR35? 
Up until April 2017, it was up to the individual worker to decide whether they were inside or outside IR35. However, IR35 legislation then changed. From 6 April 2021, the 'off-payroll' rules have been extended to include the private sector. Now, following the introduction of new rules for &lsquo;off-payroll&rsquo; workers, the call for public or private sector workers rests solely with the public or private body for whom the work is carried out. For example, if a locum doctor works in an NHS hospital, it falls to the NHS Trust in charge of that hospital to determine whether that healthcare professional is inside or outside IR35. End clients have been known to provide HMRC with incorrect information regarding their employee&rsquo;s IR35 status. Whilst these cases are rare, it is advisable to have your contract independently reviewed for IR35 compliance. Alternatively, you can use the &lsquo;Check Employment Status for Tax&rsquo; (CEST) tool.
Are umbrella companies inside IR35? 
The IR35 legislation is only applicable if you are working through a limited company. Where the umbrella company pays your limited company, the IR35 legislation applies and if you are inside, the umbrella should deduct tax and National Insurance. Where you are paid directly by an umbrella, you are an employee of that umbrella and IR35 does not apply. The umbrella will deduct the relevant tax and National Insurance from you as an employee.
What is an IR35 enquiry? 
An IR35 enquiry is a case brought forward by HMRC and occurs when a worker is found to be wrongfully claiming to be outside IR35 when, in fact, they are inside IR35. HMRC will pursue the guilty party in an effort to recoup lost income tax and National Insurance contributions. Additional interest and a potential penalty fee may also be deemed payable. For those found culpable, these charges can often stretch to tens of thousands of pounds.

Some areas in the UK are highly sought after and can increase competition for the same roles. As you build your confidence and experience you can move closer to your ideal location and role with more knowledge under your belt.

Psychiatry consultants have varied and diverse roles. And from leading a team of multidisciplinary professionals to helping rehabilitate individuals, this diversity is what Dr Baruah enjoys most every day. We asked Dr Baruah a few questions about her role as a psychiatrist, including what it entails and how working locum shifts works for her.
How long have you been a psychiatrist and why did you want to become one?
&ldquo;I&rsquo;ve been in psychiatry for more than 17 years. Before that, I worked in a few different fields, including forensic medicine and neurology. It was only when I started learning about neuropsychiatric disorders that I considered psychiatry more closely. I was always interested in brain disorders and that led me into forensic psychiatry.&rdquo;
How did you become a psychiatry consultant?
&ldquo;When I became a registered practitioner in 2001, I wanted to find out which areas of practice were best suited for me, so I went into neurology. As my interest in forensic psychiatry grew and I became more experienced, I took on my current role as a forensic community consultant.&rdquo;
What does being a psychiatry consultant entail?
&ldquo;The role of a consultant is to lead a multidisciplinary team and to decide how best to treat a patient. Providing care isn&rsquo;t just a doctor&rsquo;s responsibility. We are a team of nurses, occupational therapists, psychologists and care workers, and my role is to bring these different skills together to help patients overcome sometimes complex and challenging issues.
Usually, this means I&rsquo;m based at a hospital, but as a forensic community consultant, I&rsquo;m frequently in the community, especially where patients have been moved into local care settings.&rdquo;
What do you enjoy most about being a psychiatry consultant?
&ldquo;Making a difference to people&rsquo;s lives is very rewarding. Everything we do is about helping our patients overcome complex disorders and seeing them succeed is always very satisfying.&rdquo;
What are the challenges of being a psychiatry consultant?
&ldquo;It can be extremely hard work. You need a lot of specialist knowledge and to continually keep up to date with changes to our practice. Some of our patients also come with very difficult backgrounds and issues, and there are also challenges involved with leading a team. It can be a challenging role, but it makes the rewards all the more enjoyable.&rdquo;
What are the most important skills for a psychiatry consultant?
&ldquo;You need a lot of medical knowledge, and you need to be continually refreshing this knowledge as the field changes. Another important skill is teamwork as leading a team comes with its own challenges. Then I would say perseverance. It is a challenging role that involves a lot of hard work so being able to persevere is important.&rdquo;
Why did you choose to work locum rather than taking up a permanent role?
&ldquo;Working locum gives me more flexibility. I enjoy having a range of different cases and using my varied experience, and locum allows me to do that. In a permanent role, I&rsquo;d feel stagnant, so working in this way helps me balance the diversity of my experience with flexible working.&rdquo;
What were your first experiences with Global Medics and how have you found working with them?
&ldquo;I first started working with Global Medics in 2011. After working a few more permanent roles I got back in touch in 2015 and have stayed ever since.

Finally, what advice do you have for anybody considering becoming a psychiatry consultant?
&ldquo;My main advice is to really think about it. It does come with challenges and can be hard work, so isn&rsquo;t necessarily for everyone. But if you&rsquo;re prepared to take up the challenge, it can be an extremely rewarding role.&rdquo;
